So why does the patient keep saying they feel worse?
This tension—between what metrics report and what patients experience—is not a failure of data collection. It is a structural feature of how modern pharmacotherapy is designed, studied, and evaluated. Understanding it is not optional for clinicians who aim to prescribe with genuine precision.

Building a systematic check for functional deterioration into standard prescribing practice does not require lengthy consultations or complex instrumentation. When a patient's functional status is declining despite favorable biomarkers, the clinical imperative is not to dismiss the patient's experience or to abandon the medication uncritically. It is to reopen the goals-of-care conversation with appropriate depth.

With colleagues, presenting quality-of-life data alongside biomarker data signals that both dimensions are legitimate objects of clinical attention, not competing concerns.
Documentation practices matter here as well. Noting functional status, patient-reported symptoms, and the rationale for any deviation from standard targets creates a defensible, patient-centered clinical record.
Precision That Includes the Person
The promise of precision medicine is that treatment can be tailored to the individual rather than applied uniformly across a population. But that promise is only partially fulfilled when individualization stops at the genomic or biochemical level. True precision requires incorporating the patient's functional experience, their values, and the real-world consequences of the medications they take.
When a prescriber recognizes that a drug is winning on the lab report while losing in the patient's daily life, they are not identifying a failure of pharmacology. They are identifying an opportunity—to recalibrate, to communicate, and to prescribe with the kind of clinical judgment that numbers alone will never be able to supply.
